This section provides essential forms and documents for all graduate student clubs, as well as helpful links to Baruch campus resources. Please review and use the materials below as needed.
- Baruch Justification Form – Required for event purchases; includes event and item descriptions.
- GSA Line Change Form – Used for re-categorizing budget items (e.g., Refreshments, Supplies, Equipment).
- GSA Program Change Form – Submit changes to a previously approved program plan or event.
- GSA Program Name Change Form – Use this form to request a change to your event or program name.
- GSA Budget Appeals Form – Request additional funding or appeal a previous budget decision.
- MPR 1-107 Room Setup Template – Required for reserving and organizing events in NVC Room 1-107.
- GSA Off-Campus Travel Form – Checklist and form required for any off-campus travel activities.
